Share your thoughts and experiences of community services in Warwickshire
Health and care providers across Warwickshire are now working together to develop a plan for providing support and care closer to where people live, aiming to ensure that local needs are at the heart of their decisions and plans. We would like to know:
- Do health and care providers need to consider delivering things differently to help and support people across the different parts of Warwickshire in the ways they need?
- Should care be delivered closer to the places people call home to help keep them well and where possible, out of hospital?
- Do health, care, and community & voluntary organisations work well together?
Talk to us to help shape health and care that works for you and your community.
Your anonymised and themed feedback will be shared with SWFT (South Warwickshire University NHS Foundation Trust) and local health and care partners, to help shape health and care that works for your area, aiming to ensure that everyone has a chance to say what is needed for themselves and their community. The survey will be open throughout summer 2025.
